Application for an Amended Federal Firearms License (OMB Number 1140–0040)

The Paperwork Reduction Act of 1995 requires agencies to publish proposed information collections in the Federal Register for public comment before an agency can use the process (generally a form) to collect information from the public. As required by the Paperwork Reduction Act, this notice provides 60 days for public comment on the revision of an existing collection. he proposed information collection pertains to the ATF Form 5300.38 which is used by existing Federal firearms licensees to change the business address of the license and certify compliance with the provisions of the law for the new address. The form is also used for changes of trade or business name, changes of mailing address, changes of contact information, changes of hours of operation/availability, and allows for licensees to indicate any changes of business structure.
