eForm Access Request (OMB Number 1140–0087)

The Paperwork Reduction Act of 1995 requires agencies to publish proposed information collections in the Federal Register for public comment before an agency can use the process (generally a form) to collect information from the public. The proposed information collection was previously published in the Federal Register on Jan. 20, 2015. As required by the Paperwork Reduction Act, this notice provides an additional 30 days for public comment on the proposed extension of the existing collection. The proposed information collection requires respondents to complete the eForm Access Request form in order to receive a user ID and password to obtain access to ATF’s eForm System. This information is used by the government to verify the identity of end users prior to issuing passwords.
