New Collection; Adverse Information Suitability Request—ATF Form 3252.12

Any individual currently serving a confidential informant (CI) for ATF must provide their personally identifiable information. ATF will utilize the information to verify the identity of the individual and conduct indices checks. Respondents include members of the public who are presently serving as a CI for ATF.
